docs: 更新模块名称变更记录至CHANGELOG

更新CHANGELOG.md，记录将"工具箱"模块更名为"客户服务"的变更。此修改已在首页网格导航和设计文档中同步实施，仅涉及显示名称优化，不涉及功能变更。
